Research Cooperation Application Form

• ①　Online application for research collaboration from the administrator to the researcher
• ②　 The researcher will send explanatory materials and other information to the administrator who applied for the program by e-mail and obtain his/her approval.
• ③　The administrator who applied from the researcher will be notified how to access e-learning.
• ④　Please distribute the “Request for Research Cooperation” explanatory material for staff from the manager, post it in the office, and then provide information on how to access the e-learning system.
• ⑤　Each staff member will learn on his/her own via e-learning.
• ⑥　Each staff member will complete the online survey on his/her own.
